Butterbur is a large-leaved perennial plant native to Europe and Asia, showing promise for migraine prevention and allergic rhinitis. However, the plant naturally contains pyrrolizidine alkaloids (PAs) that are highly toxic to the liver and can cause cancer. Only PA-free standardized extracts should ever be used. Raw plant material and non-certified products are extremely dangerous and should be avoided completely.

The large leaves were used to wrap butter in warm weather, giving the plant its common name. Traditional uses included treating headaches, asthma, and urinary tract issues. However, these traditional preparations contained dangerous pyrrolizidine alkaloids that cause liver damage and cancer. Modern understanding has led to the development of PA-free extracts that retain therapeutic benefits while removing toxic compounds. Germany, the UK, and other countries have restricted or banned non-PA-free butterbur products.
