Overview

Butterbur is a large-leaved perennial plant native to Europe and Asia, showing promise for migraine prevention and allergic rhinitis. However, the plant naturally contains pyrrolizidine alkaloids (PAs) that are highly toxic to the liver and can cause cancer. Only PA-free standardized extracts should ever be used. Raw plant material and non-certified products are extremely dangerous and should be avoided completely.

Traditional Use

Butterbur has been used in European folk medicine for centuries to treat plague, fever, coughs, and wounds. The large leaves were used to wrap butter in warm weather, giving the plant its common name. Traditional uses included treating headaches, asthma, and urinary tract issues. However, these traditional preparations contained dangerous pyrrolizidine alkaloids that cause liver damage and cancer. Modern understanding has led to the development of PA-free extracts that retain therapeutic benefits while removing toxic compounds. Germany, the UK, and other countries have restricted or banned non-PA-free butterbur products.

Key Active Compounds

Petasin and Isopetasin

Sesquiterpenes with anti-inflammatory and antispasmodic properties, believed responsible for migraine prevention and allergy relief.

Pyrrolizidine Alkaloids (PAs)

TOXIC compounds naturally present in butterbur that cause severe liver damage, veno-occlusive disease, and cancer. Must be completely removed for safe use.

Flavonoids

Antioxidant compounds that may contribute to anti-inflammatory effects.

Evidence-Based Benefits

Migraine Prevention

Strong

Clinical trials show PA-free butterbur extract (Petadolex) significantly reduces migraine frequency and severity. Effects comparable to some pharmaceutical preventive medications. Recognized by American Academy of Neurology as effective.

Allergic Rhinitis Relief

Moderate

Studies demonstrate PA-free butterbur can reduce symptoms of seasonal allergies (hay fever) as effectively as some antihistamines, without causing drowsiness.

Asthma Support

Traditional use and some preliminary research suggest butterbur may help reduce asthma symptoms and improve lung function, though more research is needed.

Common Preparation Methods

PA-Free Standardized Extract ONLY

Use only commercial butterbur products certified as PA-free (such as Petadolex brand), standardized to 15% petasins.

Dosage: 50-75mg twice daily for migraine prevention; 50mg 2-3 times daily for allergies

CRITICAL: Only use products certified PA-free and tested by independent labs. Raw plant material and uncertified products are TOXIC. Effects for migraines may take 4-8 weeks.
